Job summary

VitalCaring Group seeks a Home Health Physical Therapist to deliver patient-centered care in home settings. You will evaluate functional status, develop individualized care plans, provide hands-on therapy, and educate patients and caregivers to support independence.

You’ll monitor progress, coordinate with the care team, supervise PTAs where allowed, document in the EMR, and participate in team meetings. This autonomous, relationship-driven role offers meaningful impact in patients' homes.

Qualifications

• DPT or Master's in Physical Therapy and state licensure.
• 1–2 years of experience; home health preferred.
• Current CPR certification and valid driver’s license.
• Excellent communication and patient education abilities.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety.
• Monitor progress, adjust plans, and participate in discharge planning.
• Document care accurately in the EMR and participate in team meetings.
• Supervise PTAs in line with state guidelines.
