Job summary

Providence at Home with Compassus in Eureka, CA seeks a Physical Therapist to perform home health evaluations, develop treatment plans, implement therapies, and manage discharge planning as part of a multidisciplinary team.

You will supervise PT assistants, coordinate with physicians and staff, and educate patients and families for home-based care. Valid California Physical Therapist license required; BLS within 30 days of hire; Bachelor's degree in PT.

Qualifications

• Required: Bachelor's Degree in Physical Therapy from an institution approved by the state board.
• Preferred: 1 year Therapy with adult physical disabilities.

Responsibilities

• Performs comprehensive patient assessments including ROM, manual muscle, gait, and functional analysis to establish and update treatments.
• Plans and administers physician-ordered physical therapy programs to restore function and prevent disability.
• Records and evaluates assessment findings, treatment plans, and progress notes to guide care and meet regulatory requirements.
• Administers therapeutic exercises and activities using equipment and assistive devices.
• Provides treatments involving physical agents as prescribed.
• Instructs patients and families in safe use of therapy equipment and exercises at home.
• Collaborates with physicians and team members to coordinate care and adjust treatment programs.
• Identifies need for referrals and facilitates coordination of services to support goals.
• Supervises licensed physical therapy assistants and aides to ensure quality care.
