Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to provide home health services to adults in Oakland and surrounding areas as an Independent Contractor. You will assess ADLs, design treatment plans, and collaborate with families and healthcare teams to improve safety and functional independence.

Qualifications

• Ability and enthusiasm to work with patients of all ages.
• Licensed and registered Occupational Therapist in the state.
• Graduation from an accredited Occupational Therapy program.
• Bilingual abilities are a plus.

Responsibilities

• Assess and evaluate patient care needs related to function, ADLs, fine motor coordination, home safety, and adaptive equipment.
• Develop and implement treatment plans to improve wellbeing.
• Evaluate patient progress and adjust care as needed.
• Coordinate with other health care team members and educate patients and families.
