Job summary

Achieve Behavior Alliance Plus, LLC is seeking Home & Community Support Professionals (HCSP) in Muncie, Indiana. This independent contractor role offers flexible daytime, evening hours and weekends with in-home/community-based visits and a potential full-time caseload.

As a DSP contractor, you provide person-centered support, assist with daily living activities, and help participants build skills and community participation under the HCSP Coordinator.

Qualifications

• Must be 18+ years old and able to obtain required certifications.
• Valid driver’s license with reliable transportation.
• Clean background check.
• Experience with individuals with disabilities preferred.
• Able to work independently and with a team.

Responsibilities

• Provide person-centered, direct care support to individuals with intellectual and developmental disabilities.
• Assist with daily living activities including meal preparation, personal hygiene, mobility support, and independence-building tasks.
• Follow each individual’s support plan to promote skill development, communication, and community participation.
• Maintain a safe, respectful environment while protecting dignity and privacy.
• Observe, document, and report changes in health or well-being.
• Complete accurate service documentation and session notes.
• Coordinate schedules directly with individuals and families.
• Work independently while collaborating with a support team.
• Provide meaningful work that makes a positive impact.
