Job summary

FeldCare Connects is seeking an enthusiastic Physical Therapist to deliver home healthcare to patients across Eureka and surrounding areas.

As an independent contractor, you control your schedule and coverage areas, with compensation based on per-visit rates. You will work in patients' homes, documenting progress in an EMR and coordinating care with our team to ensure quality outcomes.

Qualifications

• Ability and enthusiasm to work with patients of all ages.
• Current Physical Therapy license issued by the state board.
• Completion of an accredited Physical Therapist program.
• Reliable personal transportation.
• Smartphone required.
• Bilingual a plus (preferred).

Responsibilities

• Deliver quality home healthcare to patients of all ages.
• Observe, record, and report patient responses to treatment; adjust plan of care as needed.
• Develop and implement the plan of care with patient and family.
• Maintain accurate documentation in the Electronic Medical Record system.
• Coordinate with FeldCare Connects team for scheduling and care coordination.
