Job summary

FeldCare Connects seeks a self-motivated Physical Therapist to provide quality home healthcare to patients of all ages in Dinuba, CA and surrounding areas. You will schedule visits, determine plans of care, and document patient responses using Cliniconnects and an EMR system.

Qualifications

• Active Physical Therapy license in CA.
• Experience delivering PT in home health settings preferred.
• Must have reliable transportation and smartphone.

Responsibilities

• Deliver one-on-one physical therapy at patients' homes.
• Assess and document patient progress during visits.
• Coordinate with home health agencies for referrals and care coordination.
