Job summary

You will support individuals in accessing resources, plan engaging activities, accompany them on community outings, and document progress. We value teamwork and dedication to dignity.

Responsibilities

• Provide person-centered instruction through planned community activities.
• Support individuals in accessing community resources, planning engaging activities, and strengthening their skill sets.
• Take individuals on community outings—whether for appointments, errands, or just to enjoy the day.
• Track and document progress, capturing accurate and concise case notes.
• Redirect inappropriate behaviors and facilitate positive resolutions to challenges.
• Work collaboratively with a team of professionals to ensure comprehensive support.
