Job summary

HeartShare seeks a Vocational and Community Support Specialist to empower individuals with intellectual and developmental disabilities toward independence, community participation, and meaningful employment. The role blends community habilitation and supported employment with individualized training, coaching, advocacy, and travel within home, community, and job sites.

You will work with people, families, and multidisciplinary teams to implement Life Plans and Employment objectives, promoting

Qualifications

• High School Diploma or GED required; associate's or bachelor's degree preferred.
• Experience supporting individuals with IDD preferred.
• Experience in vocational services, job coaching or related human services preferred.
• Strong communication, interpersonal, organizational, and documentation skills.
• Ability to work independently and collaboratively in community-based environments.
• Knowledge of person-centered planning and supported employment practices preferred.
• Valid driver's license and willingness to travel throughout the community; Bronx travel 2-3 times weekly.

Responsibilities

• Provide individualized instruction to develop daily living, social, communication, and community integration skills.
• Support Life Plan, Community Habilitation Plan, and Supported Employment goals.
• Provide job coaching and vocational support in supported employment services.
• Conduct regular job-site visits to monitor progress and provide coaching.
• Collaborate with employers to identify improvements and outcomes.
• Advocate and coordinate with families, employers, and external agencies.
