Job summary

HeartShare in New York, NY, seeks a Vocational and Community Support Specialist to empower individuals with intellectual and developmental disabilities through individualized instruction, job coaching, and advocacy. The role blends community habilitation with supported employment to help clients achieve independence and meaningful employment.

Qualifications

• High School Diploma or GED required; Associate's or Bachelor's in Human Services, Psychology, Education, Rehabilitation, Social Work, or related field preferred.
• Experience supporting individuals with intellectual and developmental disabilities preferred.
• Experience in vocational services, job coaching, community habilitation, workforce development, or related human services preferred.
• Strong communication, interpersonal, organizational, and documentation skills.
• Ability to work independently and collaboratively in community-based environments.
• Knowledge of person-centered planning and supported employment practices preferred.
• Valid driver's license and ability to travel throughout the community as required.
• Willing to travel to the Bronx 2-3 times a week.

Responsibilities

• Provide individualized instruction to develop daily living, socialization, and community integration skills.
• Implement activities aligned with Life Plan and Community Habilitation Plan.
• Support participation in recreational, educational, vocational, and community activities.
• Promote inclusion, self-advocacy, and relationship-building.
• Assist in travel training and using Access-A-Ride for job commutes.
• Provide job coaching and vocational support in supported employment.
• Monitor progress with regular site visits and coaching.
• Collaborate with employers to ensure successful employment outcomes.
• Advocate and coordinate with families and agencies.
• Maintain documentation and compliance.
