Job summary

Groups Recover Together is seeking a Recovery Support Coordinator in Manchester, NH. This in-person role supports 160-180 members with health-related social needs, care coordination, and wellness across clinical and community settings.

You’ll collaborate with a multidisciplinary team, engage members with motivational interviewing, and connect them to housing, insurance, and community resources. Monday–Friday, 8:30–5:00, $46,985 annually.

Qualifications

• High school diploma required; related degrees preferred.
• Two years’ experience in case management or care coordination preferred.
• Experience with substance use or mental health challenges preferred.
• Valid driver’s license and vehicle access if travel required.
• Experience using Electronic Health Records (EHR).

Responsibilities

• Collaborate with multidisciplinary care teams to ensure high-quality, member-centered care.
• Build trusted relationships to support recovery goals.
• Provide coaching, coping skills, and engagement in recovery activities.
• Assist with reengagement and outreach for unengaged members.
• Navigate insurance, housing, transportation, and community resources.
• Manage referrals and documentation for transitions of care.
• Promote recovery-positive language and community integration.
• Support scheduling, group logistics, and EHR documentation.
