Sweetwater Care is seeking a high-performing Patient Care Liaison for the Ohio market to own relationships with hospitals, physicians, and care teams. This in-person role focuses on driving census, expanding referrals, and delivering timely admissions across our SNFs.

You will operate autonomously in the field, with leadership support and opportunities to mentor others, aiming to maximize patient placement and service excellence in post-acute care.

Qualifications

• 2+ years in hospital liaison, healthcare marketing, or post-acute business development.
• Proven ability to build referral relationships and drive admissions.
• Strong understanding of skilled nursing and post-acute care.
• Confidence, accountability, and ability to work independently in the field.
• A mindset focused on solutions, speed, and service excellence.

Responsibilities

• Own and grow your assigned Ohio territory across Skilled Nursing Facilities.
• Build and strengthen referral pipelines with hospitals, physicians, case managers, and payors.
• Convert referrals into admissions through strong clinical positioning and responsiveness.
• Identify gaps in the market and create strategies to increase census and capture rate.
• Partner with facility teams to align on goals, messaging, and execution.
• Mentor and support business development efforts across the region.
