Responsibilities

• Manage an assigned caseload of patients and their caregiver needs.
• Match caregivers with patients based on family needs and scheduling.
• Coordinate schedules between patients, families, and caregivers.
• Re-staff cases when caregivers cancel or are unavailable.
• Identify available caregivers and fill open shifts as efficiently as possible.
• Maintain regular communication with patients, families, and caregivers.
• Respond to scheduling changes, call-outs, and other time-sensitive issues.
• Follow up with families to ensure appropriate caregiver coverage.
• Build positive working relationships with caregivers and families.
• Maintain accurate records of case assignments, schedules, changes, and communications.
• Keep internal systems updated with current patient and caregiver information.
• Work closely with internal teams to resolve staffing and scheduling challenges.
• Escalate complex case issues to management when necessary.
• Help ensure continuity of care by minimizing gaps in caregiver coverage.
• Manage multiple cases and priorities while maintaining a high level of professionalism and responsiveness.
