Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Freeport, IL. The role is PRN, focused on planning, developing, and directing physical therapy services in accordance with federal, state, and local standards.

You will provide therapy services, document and bill per procedures, and collaborate with an interdisciplinary team to ensure quality resident care while maintaining CMS-compliant documentation.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Screen residents referred to physical therapy to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve identified goals.
• Communicate evaluation findings, treatment plan and recommendations to residents, families, and interdisciplinary team.
• Complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems.
• Perform other duties as assigned to fulfill responsibilities of the position.
