Job summary

Creative Solutions in Healthcare, Inc. is seeking a Full-Time Speech-Language Pathologist to join our in-house team serving residents through person-centered care for positive outcomes.

The role involves evaluations of swallowing and cognition, education for residents and caregivers, and development of individualized treatment plans including AAC support, with a focus on independence and accuracy in documentation.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to increase client’s independence.
• Provide AAC systems for individuals with expressive/language disorders.
• Educate residents and caregivers.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Provide accurate and timely billing records and supporting clinical documentation.
• Observe confidentiality and safeguard patient information.
