Job summary

Creative Solutions In HealthCare is seeking a full-time Speech-Language Pathologist to join our in-house team serving residents through person-centered care to achieve positive outcomes.

The SLP will evaluate swallowing and cognitive deficits, develop individualized treatment plans, provide AAC systems, and educate residents and caregivers, while collaborating with other professionals and ensuring compliant documentation.

Qualifications

• Active Texas license in Speech-Language Pathology.
• In good standing with all regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' conditions for swallowing or cognitive deficits.
• Help residents in their goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech and language therapy assessments and develop individualized treatment plans.
• Provide AAC systems for individuals with expressive/language disorders.
• Provide training and education to clients, family, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Demonstrate competent technical skills according to current Speech Therapy standards of practice.
• Communicate changes in resident’s condition.
• Provide accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other staff.
• Assure resident abuse or neglect does not occur.
• Observe confidentiality and safeguards for patient information.
• Complies with federal, state, and local legal and professional requirements.
