Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Speech Language Pathologist (SLP) to join our in-house team serving residents with person-centered care to achieve positive outcomes.

The position involves evaluating swallowing and cognitive deficits, delivering skilled therapy, educating residents and caregivers, and coordinating with physicians and other professionals. In-house rehab and flexible schedules are offered to qualified applicants.

Qualifications

• Active Texas license in Speech-Language Pathology
• In good standing with regulatory agencies and licensing boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different personalities

Responsibilities

• Evaluate residents' conditions for swallowing or cognitive deficits
• Assist residents in improving cognition or swallowing
• Educate residents and caregivers
• Provide skilled, medically necessary speech and language therapy assessments and develop individualized treatment plans
• Provide AAC systems for severe expressive/language disorders
• Train clients, families, caregivers, and other professionals
• Coordinate with other professionals for diagnostic swallowing procedures as ordered
• Demonstrate competence in speech therapy standards of practice
• Communicate changes in residents' condition
• Provide billing records and documentation
• Respond to family requests promptly
• Coordinate training with Restorative Nursing and other staff
• Ensure no resident abuse or neglect
• Observe confidentiality of patient information
• Comply with laws and professional requirements
