Creative Solutions In HealthCare in Houston, TX is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care. You will evaluate swallowing and cognitive function, develop individualized treatment plans, and implement therapies to improve independence.

Responsibilities include AAC provision, education for residents and caregivers, and collaborating with healthcare professionals.

Qualifikationen

• Active/Valid Texas license in Speech-Language Pathology
• In good standing with all regulatory agencies and licensing boards
• SNF/Long term care experience preferred
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred
• Ability to manage patients with different types of personalities

Aufgaben

• Evaluate residents' conditions for swallowing or cognitive deficits
• Assist residents to improve cognition or swallowing
• Educate residents and caregivers
• Provide skilled, medically necessary speech language therapy assessments and develop individualized treatment plans
• Develop and implement treatment plans to increase independence and functioning
• Provide AAC systems for individuals with expressive/language disorders
• Train and educate clients, families, caregivers, and other professionals as a subject matter expert
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician
• Demonstrate competent technical skills according to current Speech Therapy standards of practice
• Communicate changes in resident’s condition
• Provide accurate billing records and clinical documentation
• Respond to family requests and concerns timely
• Coordinate and provide training with Restorative Nursing and other Care staff
• Ensure resident safety and confidentiality of information
• Comply with federal, state, and local laws and professional requirements
