Job summary

Creative Solutions In HealthCare seeks a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care. You will evaluate swallowing and cognitive functions, plan and implement tailored therapy, and support families and staff.

The role requires an active Texas SLP license, with SNF/long-term care experience preferred. Knowledge of Net Health EHR is beneficial, and the organization offers comprehensive benefits and flexible scheduling.

Qualifications

• Active/Valid Texas license in Speech-Language Pathology is required.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents to improve cognition or swallowing.
• Educate residents and caregivers.
• Develop and implement individualized treatment plans for independence.
• Provide AAC systems for severe expressive/language disorders.
• Train clients, families, and caregivers as a subject matter expert.
• Coordinate with other professionals for diagnostic swallowing procedures.
• Maintain documentation and billing records.
