Job summary

Creative Solutions in Healthcare, Inc. is seeking a full-time Speech-Language Pathologist (SLP) to join our in-house rehab team serving residents through person-centered care and evidence-based therapy.

You will evaluate swallowing and cognitive needs, develop treatment plans, provide AAC support, educate families, and collaborate with caregivers and other clinicians to improve residents' independence. Texas licensure required; SNF/long-term care experience preferred; we offer comprehensive

Qualifications

• Active/Valid Texas license in Speech-Language Pathology.
• In good standing with all regulatory agencies and licensing boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Assist residents in goals to improve cognition or swallowing.
• Educate residents and caregivers.
• Provide skilled, medically necessary speech therapy assessments and develop/implement individualized treatment plans.
• Provide AAC systems for expressive/language disorders.
• Provide training and education to clients, family, caregivers, and other professionals.
• Coordinate with other professionals for diagnostic swallowing procedures as ordered by physician.
• Demonstrate competent technical skills per current speech therapy standards.
• Communicate changes in resident's condition.
• Provide accurate billing records and supporting documentation.
• Respond to family requests and concerns timely.
• Coordinate training with Restorative Nursing and other care staff.
• Ensure no resident abuse or neglect occurs.
• Observe confidentiality and safeguards for patient information.
• Comply with legal and professional requirements.
