Job summary

Creative Solutions In HealthCare in Texas is seeking a Full-Time Speech Language Pathologist to join our in-house rehab team serving residents with person-centered care to achieve positive outcomes.

The SLP will evaluate swallowing and cognitive function, develop individualized treatment plans, implement AAC systems when needed, and coordinate with caregivers and other professionals. We welcome new grads; competitive benefits and flexible schedules are offered.

Qualifications

• Active Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies and boards.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different personalities.

Responsibilities

• Evaluate Residents conditions for swallowing or cognitive deficits.
• Help Residents in their goals to improve Cognition or Swallowing.
• Education for Residents and Caregivers.
• Provide skilled, medically necessary speech and language therapy assessments and develop/implement individualized treatment plans.
• Provide AAC systems for individuals with expressive/language disorders.
• Provide training and education to clients, family, caregivers, and other professionals.
• Coordinate and collaborate with other professionals for diagnostic swallowing procedures when ordered by physician.
• Demonstrate competent clinical skills per Speech Therapy standards.
• Communicate changes in resident’s condition.
• Provide accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other Care staff.
• Ensure no resident abuse or neglect; safeguard confidentiality and comply with laws.
