Job summary

Creative Solutions in Healthcare, Inc. is seeking a dedicated Physical Therapist (PT) for an in-house rehab team. The role focuses on patient-centered care to improve mobility, strength, and overall quality of life for residents in a SNF or long-term care setting.

The PT will assess, plan, and implement individualized treatment programs, collaborate with an interdisciplinary team, and ensure compliance with care plans and regulations. Hiring flexibility includes full-time or PRN positions.

Qualifications

• Active and valid Texas Physical Therapist license
• In good standing with regulatory agencies and licensing boards
• SNF or long-term care experience preferred
• Knowledge of current physical therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments to relieve pain and restore function
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate
• Provide clinical support and education to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab
