Autumn Leaves Memory Care in Texas is seeking a dedicated Physical Therapist Assistant (PTA) to join our in-house rehabilitation team serving skilled nursing residents.

You will assist with rehabilitation plans, coordinate care with the broader team, and ensure compassionate, high-quality treatment. A Texas PTA license and EMR experience are preferred; flexibility and strong communication are valued.

Qualifications

• PTA license required in Texas.
• Experience in Skilled Nursing or long term care preferred.
• Experience with EMR required.
• Commitment to quality patient care.

Responsibilities

• Assist with rehabilitation of patients with emotional, physical or mental disabilities.
• Assist with implementation of physical therapy care plans.
• Assist with coordination of care with the entire care team.
• Communicate effectively with patients, caregivers, healthcare providers, office staff and other team members.
