Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapist to join our in-house skilled nursing facility partner in Le Center, MN.

You will provide PT services, develop treatment plans, document in NetHealth EMR, and collaborate with residents, families, and the care team. Requires a BS in Physical Therapy and an active PT license in Minnesota; ongoing CE is encouraged. Part-time or PRN schedule with competitive pay.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Conduct regular screenings to determine need for intervention or treatment.
• Evaluate residents to obtain data for planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate evaluation findings and plans to residents, families and the care team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
