Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Le Center, MN. This part-time or PRN role focuses on planning, directing and delivering physical therapy in compliance with CMS and local standards.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Develop and implement treatment plans with short and long-term goals; communicate findings to residents and families.
• Conduct screenings to determine need for intervention and evaluate residents for treatment planning.
