Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Worthington, MN on a PRN basis. The role requires delivering high-quality therapy, evaluating residents, and coordinating with the care team to optimize outcomes.

The successful candidate will perform assessments, develop treatment plans, and ensure CMS-compliant documentation in NetHealth EMR, while upholding professional standards and ethical care.

Qualifications

• Must have a Bachelor of Science in Physical Therapy from an accredited program.
• Must hold an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per established standards and facility procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop short- and long-term treatment plans with clear goals and methods.
• Interpret evaluation findings and communicate with residents, families, and the care team.
• Complete compliant medical documentation per CMS expectations in NetHealth EMR.
