Job summary

Evolve Therapy Services is seeking a Physical Therapist Assistant for an in-house Skilled Nursing Facility. You will work under the supervision of a Physical Therapist to provide comprehensive PT services to residents in a dedicated SNF setting.

The role involves screenings, assist with evaluations, and delivering both individual and group therapy focusing on mobility, strength, balance, gait, and ROM, following the resident’s treatment plan.

Qualifications

• Associate's degree in PTA from an accredited program.
• Active Physical Therapy Assistant license in state of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide PT services to residents under direct supervision of the PT.
• Conduct screenings to determine need for intervention.
• Assist in evaluations and in setting treatment goals.
• Contribute to treatment plans and patient progress.
• Deliver individual and group therapy per the plan, including mobility, strength, balance, gait, and ROM work.
• Assist in selecting techniques and adapting activities with guidance from the PT.
• Monitor patient responses and modify as needed.
• Assist with scheduling and coordination with the care team.
• Document in NetHealth EMR per CMS expectations.
