Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Tampa, FL. Positions available in Full Time, Part Time, or PRN to support resident care and therapy services.

Responsibilities include evaluation, treatment planning, documentation, and collaboration with the interdisciplinary team to ensure CMS and facility standards are met. Active licensure in the state is required and ongoing CE is encouraged.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of treatment programs.
• Conduct periodic screenings of residents to determine intervention needs.
• Perform timely screening of referrals to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, plans and recommendations to residents, families, and the interdisciplinary team.
