Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to join our partner facility in North Platte, NE. This in-house Skilled Nursing Facility position supports a PT under supervision to provide quality resident care and to implement treatment plans with clear goals.

Candidates must hold an Associate’s degree in Physical Therapy and an active PTA license in the practice states. Sign-on or relocation bonuses are offered, with flexible scheduling for FT/PT/PRN roles.

Qualifications

• Associate's degree in physical therapy is required.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluations and in developing treatment plans and goals.
• Implement individual and group treatments focusing on mobility, strength, balance, and functional activities.
• Document compliant and medically necessary CMS-compliant items in NetHealth EMR.
