Job summary

Evolve Therapy Services is seeking a Licensed Physical Therapy Assistant to support an in-house Skilled Nursing Facility in North Platte, NE. Full-time, part-time, or PRN positions available with a $5,000 relocation or sign-on bonus and flexible scheduling.

You will provide PT services under the supervising PT, conduct screenings, develop treatment plans, perform mobility and gait training, document in NetHealth EMR, educate residents and families, and participate in QA activities.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention.
• Assist in developing target lists with short- and long-term goals and treatment plans.
• Deliver individual and group treatment focusing on mobility, gait, transfer training, and strength.
• Assist in selecting techniques/media and adjust activities per resident performance.
• Document compliant and medically necessary care in NetHealth EMR per CMS expectations.
• Educate residents, families and staff on therapy plans and equipment use.
