Job summary

Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Full-time, part-time, or PRN with flexible scheduling and a $5,000 relocation or sign-on bonus are available.

Responsibilities include providing PT services under supervision, conducting screenings, aiding evaluation and treatment planning, and documenting CMS-ready care in NetHealth EMR. Requires an Associate's degree and an active PTA license.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Must hold an active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure and certification.
• Ongoing professional development and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct individual and group treatments across mobility, strength, gait, range of motion, and modalities as outlined by the plan.
• Assist in documenting compliant and medically necessary CMS-ready notes in NetHealth EMR.
