Job summary

Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. Under the supervision of a Physical Therapist you will provide resident-focused therapy, conduct screenings, assist in treatment planning, document in NetHealth EMR, and participate in staff education.

This full-time role includes a $5,000 sign-on bonus, with flexible scheduling including part-time or PRN options.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals.
• Assist in evaluation and development of treatment plans.
• Assist in establishing resident schedules with the Physical Therapist and team.
• Train residents and family in use of therapy equipment.
• Document compliant and medically necessary therapy interventions in EMR.
