Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in North Platte, NE. The role supports the Physical Therapist in planning and delivering therapy services to residents, with a focus on functional mobility and independence.

The PTA works under supervision, conducts screenings, assists in evaluations, and helps implement treatment plans.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluations under the direction of the Physical Therapist.
• Assist in establishing target goals and treatment plans with the PT.
• Deliver individual and group treatments focusing on strength, mobility, balance, and ROM.
• Select techniques/media and adapt as indicated with PT guidance.
• Document clinically and maintain CMS-compliant records in NetHealth EMR.
