Evolve Therapy Services, LLC is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Grand Island, NE. This PRN opportunity involves providing therapy services under the supervision of a Physical Therapist and supporting resident care.

The role requires an Associate's degree in physical therapy, an active PTA license, and commitment to ongoing professional development to maintain licensure. Collaboration with the care team and flexible scheduling are essential.

Mansioni

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduce screenings to determine need for intervention/treatment.
• Assist in evaluating and setting goals and treatment plans with the PT.
• Deliver individual and group treatments per plan, focusing on strength, mobility and gait.
• Monitor resident response and modify plans in consultation with PT.
• Schedule and coordinate resident therapy with the PT and care team.
• Document compliant and necessary CMS and EMR entries.
