Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Kenosha, Wisconsin. The role reports to a supervising Physical Therapist and involves conducting screenings, contributing to evaluation discussions, and delivering both individual and group therapies within approved treatment plans.

Responsibilities include documenting care in NetHealth EMR, coordinating with staff, and supporting resident education and ongoing progress

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and in developing treatment plans and goals with the PT.
• Implement individual and group treatment as outlined in the plan including mobility, strength, gait, balance, and ROM activities.
• Document compliant and medically necessary therapy activities in the EMR (NetHealth).

Education

Associate's degree in Physical Therapy

Active PTA license in state(s) of practice
