Job summary

Evolve Therapy Services is hiring a Physical Therapist Assistant for an in-house Skilled Nursing Facility in Milwaukee, WI. The PTA will provide therapy under the supervision of a Physical Therapist and help plan, organize, and deliver resident-centered physical therapy services.

Responsibilities include screenings, developing treatment plans with the PT, conducting individual and group treatments, and maintaining CMS-compliant documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.
• Must maintain licensure and certification through ongoing CE.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluation and development of treatment plans with the Physical Therapist.
• Participate in establishing and modifying individual resident schedules with the Physical Therapist and team.
• Train residents, families, and staff in the use of therapy equipment.
• Document CMS-compliant and medically necessary data in NetHealth EMR as required.
