Job summary

Evolve Therapy Services, LLC in Oakdale, NY is seeking a full-time Physical Therapy Assistant to support an in-house Skilled Nursing Facility. You will work under a Physical Therapist to provide resident care and therapy services.

Responsibilities include screenings, implementing treatment plans, documenting CMS-compliant notes in NetHealth EMR, and educating residents and staff. A PTA license and an accredited associate degree are required, with ongoing professional development encouraged.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education and licensure maintenance.
• Ability to work under supervision of Physical Therapist.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluations and treatment planning.
• Deliver individual and group treatments per treatment plan and activities.
• Document CMS-compliant notes in NetHealth EMR.
• Participate in resident, family, and staff education.
