Job summary

Evolve Therapy Services in Oakdale, NY is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role supports the Physical Therapist by delivering services, conducting screenings, and contributing to treatment plans to maintain high-quality resident care.

You will train residents and staff, participate in education, and ensure compliant documentation in NetHealth EMR per CMS standards. Full-time, on-site position with competitive pay and opportunities for growth.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations at appropriate intervals and assist in the evaluation under the direction of the Physical Therapist.
• Assist the Physical Therapist in identifying treatment goals and plans.
• Conduct individual and group treatment as outlined in the treatment plan, including mobility, strengthening, gait, ROM, and modalities.
• Monitor residents’ response to intervention and consult with the Physical Therapist for modifications.
• Establish and modify resident schedules with the team, and participate in education for residents, families, and staff.
• Ensure compliant and medically necessary documentation in NetHealth EMR per CMS expectations.

Education
