Evolve Therapy Services in Oakdale, NY is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility. You will provide therapy under the supervision of a Physical Therapist, assist in evaluations, help set goals, and participate in resident education and CMS-compliant documentation.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Screen residents to determine need for intervention and plan treatment.
• Assist in evaluation and treatment planning under supervision.
• Support development and modification of individual resident schedules.
• Deliver both individual and group therapy sessions per the treatment plan.
• Participate in documentation in NetHealth EMR per CMS expectations.
