Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Selma, AL. The role supports the Physical Therapist and delivers patient-centered care to residents in a collaborative setting.

Responsibilities include screenings, treatment delivery, progress documentation, and CMS-compliant record-keeping. The position offers multiple schedules (FT/PT/PRN) and opportunities for professional growth within the facility.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluations and in establishing short- and long-term treatment goals with the Physical Therapist.
• Conduct individual and group treatments covering mobility, strength, gait, ROM and modalities as outlined in the treatment plan.
• Monitor residents’ responses to intervention and consult with the Physical Therapist for modifications.
• Complete CMS-compliant documentation in NetHealth EMR systems.
