Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for our in-house Skilled Nursing Facility in Selma, AL. The role supports therapy under a supervising Physical Therapist and includes individual and group sessions, documentation, and care coordination.

The ideal candidate holds an Associate's degree in Physical Therapy, an active PTA license, and is committed to ongoing professional development.

Qualifications

• Associate degree in physical therapy from an accredited program.
• Active PTA license in state of practice.
• Ongoing continuing education to maintain licensure.
• Commitment to patient-focused care and safety.
• Ability to collaborate with therapists and care teams.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in developing treatment plans.
• Assist in targeting short- and long-term goals with the Physical Therapist.
• Deliver individual and group therapy sessions per treatment plan.
• Monitor resident response and modify care in consultation with the Physical Therapist.
• Assist in scheduling and coordinating resident treatment with the team.
• Complete compliant CMS documentation in NetHealth EMR.
