Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Selma, AL. This full-time, part-time, or PRN role delivers high-quality PT services in accordance with federal, state, and CMS guidelines.

You will screen residents, develop and implement treatment plans, document in NetHealth EMR systems, and collaborate with families and the interdisciplinary team to ensure optimal resident outcomes.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to procedures regarding documentation and billing of physical therapy services.
• Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.
• Interpret and communicate evaluation findings and treatment recommendations to residents and interdisciplinary team.
