Job summary

Evolve Therapy Services in Selma, AL seeks a dedicated Physical Therapy Assistant to support an in-house Skilled Nursing Facility, under the supervision of a Physical Therapist. This role can be Full Time, Part Time or PRN and emphasizes planning, implementing, and monitoring therapy plans with group and individual sessions, documentation in NetHealth EMR, and resident/family education.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluation under the Physical Therapist.
• Assist in developing target lists and treatment plans with short- and long-term goals.
• Deliver both individual and group treatments per the treatment plan, including mobility and gait training.
• Assist in selecting and adapting physical therapy techniques and modalities as directed.
• Monitor residents’ response and consult with the Physical Therapist for modifications.
• Schedule development with the team and adherence to treatment regimens.
• Train residents and families in equipment use and safety.
• Participate in resident, family, and staff education.
• Maintain CMS-compliant documentation in NetHealth EMR.

Education

Associate's degree in physical therapy

P.T.A. license (active) in state(s) of practice
