Job summary

Evolve Therapy Services in Selma, AL is seeking a licensed Physical Therapist to support our in-house Skilled Nursing Facility partner. This role offers full-time, part-time or PRN positions and focuses on delivering high-quality resident care through evidence-based PT services.

You will assess residents, develop treatment plans, document CMS-compliant visits in NetHealth EMR, and collaborate with an interdisciplinary team.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.
• Documentation and CMS-compliant reporting experience preferred.

Responsibilities

• Provide physical therapy services to residents per standards and productivity targets.
• Document and bill for therapy services per procedures.
• Evaluate, plan, implement, and follow up resident therapy programs.
• Screen residents regularly to determine need for intervention.
• Develop treatment plans with short and long-term goals.
• Communicate findings to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
