Evolve Therapy Services is seeking licensed Physical Therapists to join an in-house skilled nursing facility in Selma, AL. You will plan, organize, and direct physical therapy services to residents while ensuring CMS-compliant documentation and adherence to state/federal standards.

Positions available as full-time, part-time, or PRN, with competitive pay and benefits. You will collaborate with an interdisciplinary team to deliver high-quality resident care and ongoing professional development

Formación

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Commitment to ongoing professional development.

Responsabilidades

• Provide physical therapy services to residents in accordance with established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and recommendations to residents and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems.
