Job summary

Evolve Therapy Services in Wilson, NC is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The position supports resident care and CMS-compliant practice with licensure in the state.

Responsibilities include evaluating residents, developing treatment plans, documenting in NetHealth EMR, and collaborating with an interdisciplinary team. Flexible Full Time, Part Time, or PRN positions are available.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Document and bill for services according to procedures.
• Evaluate and plan resident therapy programs with interdisciplinary team.
• Screen residents regularly to determine need for intervention.
• Screen referrals promptly to determine intervention needs.
• Evaluate data for treatment planning and implementation.
• Develop short- and long-term treatment goals and methods.
• Communicate findings and plans to residents, families, and team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform other duties as required to support resident care.
