Job summary

Evolve-Therapy-Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role supports planning, organizing, and directing therapy services to ensure high-quality resident care. Full Time, Part Time, or PRN positions available with emphasis on compliant documentation.

Responsibilities include evaluation, treatment planning, interdisciplinary communication, and ongoing professional development to maintain licensure and certification.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Active PT license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Documentation and CMS/EMR compliance experience.

Responsibilities

• Provide PT services to residents per standards and procedures.
• Document and bill services in NetHealth EMR as required.
• Evaluate residents and develop treatment plans with goals.
• Screen residents regularly to determine intervention needs.
• Communicate findings and plans to families and team.
