Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in the City of Orange, NJ. This PRN role focuses on planning, directing therapy services and ensuring CMS-compliant documentation using NetHealth EMR.

Responsibilities include patient evaluation, treatment planning, and collaboration with families and the interdisciplinary team to achieve resident goals. Requires a BS in Physical Therapy and active license in applicable states, with ongoing CEU

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice.
• Adhere to documentation and billing procedures.
• Demonstrate judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods.
• Interpret and communicate findings to residents, families, and inter-disciplinary team.
