Job summary

Evolve Therapy Services, LLC in Madison, NJ is seeking a Physical Therapist for an in-house Skilled Nursing Facility. Full Time, Part Time, or PRN positions are available.

You will plan, develop, and direct Physical Therapy services to ensure high-quality resident care and adherence to CMS and company standards. Responsibilities include evaluating residents, creating treatment plans, and documenting in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Adhere to documentation and billing procedures.
• Develop and implement treatment plans with short and long-term goals.
• Conduct screenings to determine need for intervention or treatment.
• Communicate findings and recommendations to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
