Job summary

Evolve Therapy Services is hiring a compassionate Physical Therapist for an in-house Skilled Nursing Facility located in Cedar Grove, NJ. This full-time role involves planning, directing and providing physical therapy services to residents in accordance with federal, state, and local standards.

You will evaluate residents, develop treatment plans with measurable goals, and maintain CMS-compliant documentation.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and departmental procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of resident therapy programs.
• Conduct screening of residents to determine need for intervention or treatment.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment recommendations to residents and interdisciplinary team.

Education

Bachelor of Science Degree in Physical Therapy

Active Physical Therapy license in state(s) of practice

Continuing education to maintain licensure
