This full-time role focuses on planning, directing, and providing physical therapy services in accordance with federal, state, and local standards to ensure high-quality resident care.

Responsibilities include evaluation, treatment planning, progress documentation, and interdisciplinary communication.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Maintain licensure and certification through ongoing professional development.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards and protocols.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate and develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and recommendations to residents, families, and interdisciplinary teams.
• Complete CMS-compliant documentation in EMR systems (NetHealth).
